Who is Eligible for Benefits?
Associate
All Corporate, Center Management, and Full-Time hourly Associates working a minimum of 30 hours per week are eligible for Lucky Strike Entertainment’s benefits program.
Dependents
You can enroll your dependents in many of the benefits you choose for yourself. Eligible dependents include:
- Your legally married spouse
- Children up to age 26
- A newborn, natural child, or a child placed with you for adoption
- A stepchild or any other child for whom you have a legal guardianship or court-ordered custody, who receives more than one-half of his or her support from you
- Disabled children over 26 years of age
Part-Time, temporary, and contract Associates are not eligible to participate in the Lucky Strike Entertainment benefits program.
Open Enrollment is November 1st - 14th, 2024
This is your once-a-year opportunity to pick your plans or make changes to your existing coverage. The choices you make during Open Enrollment will become effective on January 1, 2025 and will remain in effect through December 31, 2025.
When you enroll you can:
- Change your current coverage, if needed
- Enroll in benefits for the first time, if newly hired or newly eligible for benefits
- Update your dependent information
- Update your beneficiary information
Qualifying Life Event
What is a Qualifying Life Event?
Certain life events provide you a 30-day window to elect benefits or change your covered dependents.
If you experience any of the scenarios below, you may be eligible to update your benefit elections mid-year:
- Marriage, divorce, or legal separation
- Birth or adoption of a child
- Commencement or termination of adoption proceedings
- Change in child’s dependent status
- Death of a dependent
- Loss or gain of other health coverage for you and/or dependents
- Change in employment status
- Change in Medicaid/Medicare eligibility for you or a dependent
- Receipt of a Qualified Medical Child Support Order
If you have questions about your eligibility or enrollment process, check with your plan administrator. Documentation is required for all dependent verification.
