To keep you healthy throughout the year, our Wellness Program provides many resources to help you along your wellness journey. As a Lucky Strike Entertainment medical plan participant, there are many ways to complete wellness activities and earn rewards!
Keep Medical Discounted Premiums with an Annual Preventive Exam
Lucky Strike Entertainment offers a $15 weekly medical premium discount ($780 annually) for associates who complete a Preventive Care Exam between May 1st and April 30th.
Preventive care includes routine well exams, screenings and immunizations intended to prevent or avoid illness or other health problems. Talk to your primary care physician about which covered preventive services are right for you. More details on covered services are available at uhc.com/preventivecare.
For associates with an enrolled spouse, the spouse must also complete the exam to maintain the discount. If both the associate and spouse complete it, the total savings is $1,560 annually. If not completed, the premium increases by $15 per week per person.
If your medical coverage starts after January 1st you automatically receive the discount for the remainder of the year but must complete the exam between May 1st and April 30th to keep it the following year.
